 Dear Parent/Guardian:

    On Saturday, October 20th , students participating in the trip to Kansas City World’s of Fun will leave the school at 6:30 a.m. SHARP and return Sunday evening on October 21st. The science club will take care of the rooms but students will need to reimburse Mr. Kuhlman for their park ticket. In addition, students need to bring enough money for food.
    We will be staying at the Super 8 in Mound City, MO.  The phone number is 660-442-4000. Mrs. Blaha will be accompanying the students as I have an outpatient procedure scheduled for that weekend. Her cell phone number is 605-491-4661. In addition, Chloe VanGerpen and Lea Wormsbecher will also be serving as chaperones and guides. The phone number for World’s of Fun is 816-454-4545. The students should check the Internet for the weather forecast for Kansas City that day so they can bring appropriate clothing. The park is open until 1:00 a.m. so they should think about this when packing clothes. It can get quite cold this time of year so students should be prepared for that.  Please read through the following stipulations as to alleviate any misunderstanding this trip is merely a vacation or party.  On the contrary, it is an opportunity for students to experience math and science in a meaningful and enjoyable way.  After reading through these stipulations, please be sure both you and your child sign at the bottom to indicate these rules are understood and agreed upon before departure.  No student will be allowed to participate on this trip without a signed form.

 1.)    Use or possession of alcohol, illegal drugs, or tobacco is prohibited. Local authorities will be called for any individual who is involved with alcohol, illegal drugs, or tobacco.

 2.)    The student’s write up for the rides on this trip will count as one test score. Students will be responsible for taking certain measurements for each ride. The write-ups are impossible to do without this information so students should do this first and then they can be free to enjoy the park on their own.

  3.)    Each student is responsible for being on time according to the day’s schedule. This includes getting to bed on time and being ready on time in the morning.

  4.)    No student is to engage in any activity that might endanger individual safety or cause property damage. Students should obey all safety warnings at the amusement park.

  5.) Students will mostly be on their own at the park. Therefore, STUDENTS WILL ALWAYS TRAVEL IN AT LEAST PAIRS!!! AT NO TIME SHOULD A STUDENT BE  BY THEMSELVES OR WITH PEOPLE FROM ANOTHER TOWN. Students should give their cell phone numbers to Mrs. Blaha

  6.) All students need to sleep in their assigned rooms. We will not be returning to our hotel until around 2:30 a.m. thus students need to be quiet as to not disturb people in adjoining rooms.

  7.)    Any violation of school district policy will result in the appropriate disciplinary action as outlined in the Avon school handbook.

 IMPORTANT NOTES:

No student is required to go on the trip or on any particular ride in order to get full credit.  An alternative activity will be provided for those who are unable to attend.

By submitting this consent form, I affirm that I give my permission for my child to attend World's of Fun Physic's trip on October 15th and 16th 2016.

 I hereby give consent to the Avon school staff to seek health services for my son/daughter for any emergency accidents or illness during the trip.

Please list any medication currently prescribed or routinely taken and any other medical information that you think I should be aware of prior to this trip.
